Certified Home Health Agency (CHHA)

The Columbia County Department of Health Certified Home Health Agency (CHHA) provides a wide range of skilled health care services to the residents of Columbia County. Our professional and experienced staff consists of Home Health Aides, Registered Nurses, Physical Therapists, Occupational Therapist, Speech Therapists and Medical Social Workers. Services are provided to patients ranging from newborn to senior adults are provided on a 24 hour, 7 days per week basis. There is an on-call administrator available after hours, on weekends and holidays to coordinated client's needs. Services are rendered regardless of ability to pay, and most insurance plans are accepted.

Our home care services assist clients in a variety of ways, many times following hospitalization. These services provide skilled care for both acute and chronic health care issues and for PRI nursing home assessments. The goal of our program is to educate the client and client's family about ways to manage their illness, prevent complications and promote wellness. In many cases our services help to prevent the need for rehospitalization and help client's remain in the comfort of their own homes, often utilizing other available community resources to augment the care and services provided to the client.

Care is provided for a designated period of time dependent on orders written by your doctor and on the amount and type of services required. Once admitted, a registered nurse will make a home visit to complete an admission assessment and will develop a plan of care with the client and client's family and the client's private physician. The plan of care will determine type, frequency and duration of services that will be provided. The client's private physician will then approve the plan of care. Each client's plan of care is specifically designed to meet his or her individualized needs.
